As a courtesy to our patients, Virginia Family Dentistry will complete and file insurance forms relative to dental services. Whenever insurance requires a predetermination of benefits before work is started, Virginia Family Dentistry will submit to the insurance company a treatment sequence with charges so that you will be completely aware of your coverage.
Please note that you need to provide us with a current insurance card for each family member for each visit.
During the course of treatment you will receive statements on a monthly basis regardless of your insurance coverage. Patients will be expected to begin payment on the projected balance at the time treatment begins.
Virginia Family Dentistry participates with most plans of the following dental insurances:
- Delta Dental Premier
- Dominion National
- United Healthcare
- United Concordia
We recommend that you verify with your insurance company ensuring that they are indeed a participating company.
Medicaid and Medicare: Virginia Family Dentistry is not a participating provider with Medicaid or Medicare plans. Should you wish to seek treatment with Virginia Family Dentistry, we will be happy to see you as a self-pay patient.
